Clinical Pharmacist for GP Practice & PCN - Remote

They invite experienced Clinical Pharmacists to join on a contractual basis, offering the opportunity to work remotely, manage your schedule, and contribute to their dynamic healthcare services.
Role Requirements:
Essential:
- GPhC registration
- Independent Prescriber Status (preferred)
- Minimum of 2 years Post Graduate experience in a GP Practice
- Proficiency in using SystemOne or EMIS
- Completion of the CPPE pathway
- Strong clinical leadership, communication, and interpersonal skills
Desirable:
- Experience in running chronic condition clinics
- Proficient in structured medication reviews and acute medicines request reviews

Key Responsibilities:
- Conduct comprehensive medication reviews to optimize patient treatment plans
- Review and manage acute medication requests with a focus on patient safety and efficacy
- Efficiently process hospital discharge summaries and clinic letters, ensuring seamless patient care transitions
- Offer expert support and guidance for patients managing self-limiting conditions, empowering self-care
- Support practices in achieving Quality Outcomes Framework (QOF) targets, enhancing practice performance and patient outcomes
- Play a pivotal role in enhancing health outcomes for patients with long-term conditions through collaborative care
- Undertake additional tasks as needed to support the healthcare team and patient care initiatives
